• Two proofs of legal birth name (Ex. birth certificate, social security card).
• Proof of immunization on a Tennessee School Entry Immunization Form.
• Proof of legal custody if parents are divorced.
• Copy of current physical, if the student is enrolling for the first time in a TN school.
• Proof of residence in school zone for Franklin County Schools.

• Make school attendance a priority.
• Being late to school disrupts the school day.
• Make sure to turn in notes to your child's school when they are absent.
• Students are only allowed five parent notes per school year.
• Students who have five or more unexcused absences during the school year are truant and subject to court.
• Please avoid medical appointments and extended trips when school is in session.
